Summer Childcare 2011 Registration Form  *Please submit a seperate form for each child you enroll*     **We welcome children from ages 18 months to ten years.  **Families do NOT have to be enrolled in any other Discovery Time Program to register for summer child care. **A Non-refundable Registration Fee of $50.00 to reserve a space is required for New families ONLY. Registration fees may be paid online with a credit/debit card or by check or money order to: Discovery Time School 322 S Juliana St. Suite 2 Bedford, PA. 15522 Summer Childcare is invoiced bi-weekly.  Scheduling is required to ensure that space is available. Summer Childcare Rates:      Half Day (under five hours)= $12.00 per day      Full Day   (over five hours)= $22.00 per day Please fill in all of the fields below and select the program or programs of your choice.
